Care Navigator

Salary: £12.71 per hour

Contract: Full-time

Locations: Pilgrim Primary Care Centre, Immingham & Weelsby View Health Centre, Grimsby

Care Navigators play a crucial part in helping patients get the right support, at the right time, to help manage a wide range of needs. The role is designed to help reduce inappropriate GP appointments, ensuring we have the capacity to manage those patients most at risk. Care Navigators assist and direct patients in accessing appropriate services, undertake administrative duties, maintain and monitor appointments, and more.

The Care Navigation team is a large team consisting of all ages and experience. The team are highly skilled and very hard working and strive together to achieve the best outcomes for both patients and teams.
